Sending in your forms and records is quick and easy; both online and by fax. All you need is access to a scanner to upload your forms at the link above, or you can check with your local Staple's/Office Depot stores to have all of your forms faxed over.

Meningococcal Disease
Bacterial meningitis is a very serious, potentially fatal illness. While it is fairly rare, first-year students living in residence halls have a statistically higher chance of getting the disease than young adults of the same age who aren't college freshmen living in residence halls. |

Hepatitis B
Hepatitis B is a serious infectious disease caused by a virus that attacks the liver. The highest rate of disease occurs in individuals between the ages of 20 and 49 and living in close quarters (like a residence hall) can increase the risk of exposure. There is no cure for Hepatitis B, but the infection can be prevented by vaccination. The American College Health Association (AHCA) recommends that all college students be vaccinated |

If you are from Florida and your healthcare provider is a participating physician, you can log in to Florida Shots to print a copy of your Immunization Records to submit with your Medical History form. |

All students new to USF are required to submit a signed
copy of the official USF Medical History form and submit immunization documentation for the following:
- Medical History Form: Sign the Mandatory Immunization Health History Form
- Measles 1, Measles 2, Rubella (MMR): Vaccination (2 doses after 1st birthday) OR Titer (lab work) Date & Result
- Hepatitis B-1: Vaccination OR Check the declination box OR Titer (lab work) Date & Result
- Meningitis: Menactra/MCV4 vaccination at AGE 16 OR OLDER (if living on campus) OR check the declination (if not living on campus)
- TB Screening: Tuberculosis Screening required for all International Students and U.S. born students residing at an address outside the U.S. at the time of application.
Without submitting proof of the required immunizations or signing the acceptable waivers, you will NOT be able to register for classes. Residential students will NOT receive a housing assignment without showing proof of meningitis vaccination.

Online Records Submission
To remove an IM / MH hold:
- Make sure you have completed and submitted a signed Medical History form along with your immunization records and that there is no missing information.
- Log in to your OASIS account and view your immunization records (under Registration & Records). It will list which items are missing.
- Locate records of missing items and send them to us via mail, email, fax or online submission. Or you can come into the Student Health clinic to get the vaccination shots and they will immediately be entered into your record.

USF regulations have changed in recent years and it is possible that your previously submitted records do not meet the new requirements. Log in to your OASIS account, go to Personal Records, and view what is listed. If you do see a hold, you will need to update the records listed as missing. You will need to have on file immunization documentation for: |

You can access your immunization records online by logging in to your OASIS account. Go to Personal Records, then view Immunization Records to print your records. |

Form & Records Submission
